(Boursier.com) — The group Korian announces the relocation of its Home Hospitalization establishment in Neufchâteau (Vosges), created in 2013, to the city center, within the SNCF station. Thanks to complete renovation works, the Group is rehabilitating an area of 286 m2 that has been unoccupied for several years and for which it won the call for projects launched by Sncf Gares & Connexions in 2019. These new premises allow it to integrate a teleconsultation room from where doctors will offer assisted teleconsultations.
The care team, numbering 25, includes coordinating physicians, health managers, monitoring nurses, liaison nurses, care nurses, nursing assistants and psychologists. A multidisciplinary team allows patients to be hospitalized at home, benefiting from complex, intense and technical care (mostly palliative care) that city medicine cannot support given their medical complexity.
Among its projects, the HAD of Neufchâteau will develop blood transfusion at home as well as early evaluations in EHPAD, allowing faster care and avoiding hospitalizations.
Fully associated with the territorial network of the organization of care in the western Vosges and two cantons in southern Meusien, its teams intervened with 533 patients in 2021. The increase in requests for hospitalization at home goes from elsewhere lead to a strengthening of the team with the arrival of 8 new employees by the end of 2022.
The establishment has entered into agreements with several health, medico-social and social establishments (hospitals, clinics, EHPADs, SSIADs, MAS, local health networks, mobile teams, other HADs) in Greater Nancy and Epinal. It works closely with more than 250 city health professionals in the territory (nurses and masseurs-physiotherapists, biologists, pharmacies, speech therapists, etc.), who intervene directly in the homes of patients followed by HAD.
