The medical agreement has not yet been signed, but its fate is now sealed: with the support of the Union of Liberal Doctors (SML), which, on the evening of Friday May 31, announced that it was giving “his luck” to the text, there are now four unions representing private doctors – out of six – to agree to initial the contract binding them, for the next five years, to Health Insurance. With the yes of the SML, and after that of the Federation of Doctors of France, the Confederation of French Medical Unions and MG France, the majority among general practitioners, the number of votes is sufficient for this convention, determining the conditions of exercise and remuneration for the entire profession, is finally endorsed. With a flagship measure: the revaluation of the basic consultation to 30 euros with the general practitioner, compared to 26.50 euros today.

The suspense lasted six months, punctuated by heated meetings between representatives of city medicine and the National Health Insurance Fund. And even more time, if we take into account a conventional first round, also lasting a semester, which failed in February 2023.

A few weeks ago, several unions left the discussion table to express their discontent and support a call for a strike launched by private clinics (to which they have since returned). Among them, Avenir-Spé Le Bloc, the majority among specialists: this union is the only one, to date, not to have made its position on the convention known. It must consult its members, and communicate its final decision on Monday June 3, the day before the signing session, set by Health Insurance for Tuesday June 4. Only one no, that of the French Union for Free-Union Medicine, was made official, this organization of general practitioners and specialists denouncing a convention “without vision and without ambition”carrying a “complexification shock” and a “corporalization of liberal doctors”.

What will this contract change once signed? Concerning general medicine, the first link in care, the emphasis was placed on the basic consultation, at 30 euros – a minimum alignment with inflation, according to the liberals –, but not only that: the “attending doctor” package is redesigned to better support general practitioners who assume this central role, with an effort made in particular towards practitioners who will follow patients with long-term illnesses, who are older, more fragile, etc.
