When the decision to enroll an elderly person in a nursing home is made, entry into the establishment does not happen instantly. Several weeks, or even several months in certain regions where places are insufficient, are necessary, the time to gather the necessary documents, obtain a place and organize the installation of the person.
Nursing home registration file: a medical component and an administrative component
The nursing home registration file has two parts: a medical part and an administrative part.
The medical aspect of registration in a nursing home
It must be completed by the attending physician or by another doctor (for example the hospital doctor if the person is hospitalized). It includes information on the state of health of the elderly person: medical, surgical, psychiatric history, current pathologies, current treatments, height, weight, if there is a risk of falling, aspiration, etc. The file also specifies the level of autonomy of the elderly person (can they get up on their own? Does they need help with toileting? Is they coherent?), their possible behavioral and psychological symptoms, if they needs a wheelchair or a medical bed etc.
The administrative aspect of registration in nursing homes
This part of the file aims to specify the administrative information concerning the elderly person (marital status, address etc.), if they are the subject of a legal protection measure, details on the context of the request, the person to contact about the request. The administrative aspect also makes it possible to detail any assistance available to the future resident for payment of the nursing home.
ViaTrajectoire: a platform to centralize requests
In more than 80 departments in France, registration in a nursing home is done via the ViaTrajectoire platform. To find out if the establishment that interests you is attached to it, you must go to the directory of nursing homes available on the information site for elderly people and their loved ones. The establishment’s information sheet specifies the registration procedures but also the procedure for carrying it out. Either the form refers directly to the ViaTrajectoire service, or it is necessary to download form 14732*03 request for admission to a residential establishment for dependent elderly people, then send it to the establishment concerned.
The advantage of ViaTrajectoire is that the platform allows you to complete the admission file once and send it simultaneously to several nursing homes. The site also offers a national directory of retirement homes, assistance in finding establishments meeting the applicant’s criteria and allows the secure transmission of medical data.
Registration in a nursing home: what supporting documents must be provided?
The supporting documents are not always the same and you should therefore consult the establishment’s page for a detailed list. It is not necessary to provide all of these documents to make a request on Viatrajectoire. The papers can be downloaded later.
Among the essential documents, however, we can cite: the identity document, the vital card and the certificate of vital card, the last notice of taxation or non-taxation, proof of retirement pensions, certificate of civil liability
